Office Financial Policy
We would like to thank you for choosing Guido Family Dentistry as your dental family. Excellent dental care comes not only from our dedication to your needs and comforts, but also from our dedication to maintain open and honest communication regarding our current office and financial policies.
PAYMENT Payment is expected in full at the time of service unless prior financial arrangements were previously decided. We accept cash, personal checks, all major credit cards, and flexible financing options. We also offer a 3-5% cash courtesy for payment of treatment plans over $500 one week before the first day of treatment.
All financing is submitted through our financing partner, CareCredit. They can offer no-interest payment plans for up to 12 months as well as extended, low-interest payment plans. Applications can be submitted online at www.carecredit.com or through our office.
Patients with an outstanding balance more than 90 days overdue must make arrangements for payment prior to scheduling appointments. Patients are ultimately responsible for any charges or portion thereof for which payment is denied by insurance for whatever reason.
INSURANCE It is the patient’s responsibility to provide us with current insurance information and to present an active insurance card at each visit.
Guido Family Dentistry is committed to helping our patients maximize their benefits. However, please remember that your dental insurance is your responsibility. We can estimate your benefits in good faith, but insurance policies vary greatly so they are not a guarantee of payment. As a courtesy to you, we happily manage all claim submissions along with any follow-up necessary. We also accept assignment of benefit payments from most insurance companies, helping to reduce your immediate investment in your dental care. We must stress, though, that the patient remains responsible for the total treatment fee. In the event a patient’s insurance company has not paid within 60 days, the balance may be transferred to your account. If you have any questions about how to maximize your benefits in our practice, our courteous staff is always available to help.
PAST DUE ACCOUNTS It is our policy to charge an interest rate of 1.5% monthly (18% annual percentage rate) on any past due account over 60 days.
If an account must be turned over to a collection agency, the patient’s account holder will be responsible for all costs and expenses of collection including, but not limited to, legal fees and interest accrual.
